- Heart and Soul - National Nursing Centers Consortium
- Heart and Soul is conducted at NNCC member nurse-managed health centers and other community sites. At the sites, an outreach educator runs weekly exercise programs, providing healthy snacks and teaching participants how to recognize, prevent and/or reduce high blood pressure. Participants also benefit from regular blood pressure monitoring. The outreach educator presents nutritional information monthly, combining topics such as reducing salt, fat and sugar intake.
